It is always useful to keep track of past accidents, incidents, disasters and other negative events, that occurred because of poor interaction design, even if they happened long time ago with non-digital systems, because the reasons of design-decisions are human-specific, and humans do not change as fast as evolution of technologies.
Please keep in mind, that there is never a single reason of the incident — it is always a sequence. Phrases like “poor UI killed” are oversimplification, and they are only good for tabloids and clickbait headlines, not for professional interest.
